In an encounter with an opponent- who goes first, the player or the opponent? I can't find how you determine initiative during the combat rounds.

The players should go first and the monsters go last. Which player goes first? I suppose you could roll an ability check and see who rolls highest. That's a good question and I'll upload a FAQ soon as I get more of these.

Or perhaps everyone rolls 1d6; those rolling the same number roll again?

I find with a cooperative game it's fun to have all the players go first and then the monsters. Rolling 1d6 to find the order is a fine solution.
